Bridges to Health (B2H)
This is a home and community based Medicaid service waiver program designed to provide children in foster care who have at least one of the following: significant mental health, developmental disabilities, or health care needs. We provide such individuals with services to help them live in a home or community-based setting.
The B2H program consists of the following three waivers:
- B2H for Children with Serious Emotional Disturbances (SED)
- B2H for children with Developmental Disabilities (DD)
- B2H for children who are Medically Fragile (MedF)
