Service Coordination

Medicaid Service Coordination is the active assistance offered to individuals as they navigate the community and various service systems in pursuit of the necessary desired services and supports that will assist them in achieving or maintain their personal goals. This program uses the person-centered planning approach in developing, implementing and maintaining an Individualized Service Plan (ISP). Medicaid Service Coordination promotes the concepts of informed choice, individualized services, supports and satisfaction.

Who’s eligible for this program? This program is available to individuals of all ages who have been diagnosed with a developmental disability and currently receive Medicaid benefits.
